We ask that every child has a collection password that can be used by friends/relatives who you may need to collect your child from Pre School. Staff will ask for this password from the person collecting. It is your responsibility to ensure that all friends/relatives are aware of this password.

Funding

Tapestry

Cherry Blossom Pre School uses Tapestry to track your child’s progress and development, as well as recording observations, accidents, and details of your child’s day, sharing updates, and recording bookings. It is therefore essential that each family is registered and has access to Tapestry.

Please tick to confirm that you consent to registering yourself as a Parent on Tapestry (the easiest way to do so is to download the Tapestry App on a smart phone/device). Once your child is registered with Pre School, a link will be sent to you via email.

Please carefully read through the statements below and tick the boxes to state whether you give consent for each.

Do you consent to nappy cream (Sudocrem or Bepanthen) being applied to your child if staff deem it necessary?

In the event of not being able to contact anyone authorised on this form, I give permission for staff to take my child to seek medical treatment in an emergency.

I give permission for my child to have adhesive dressings or hyper allergenic plasters administered as necessary when in Pre School. These will be administered by a trained first aider and an accident record with treatment given will be filled in.

I understand that my child will not be permitted to play outside unless he/she is adequately protected from the sun.

I give permission for Pre School staff to apply sun cream provided by Pre School if I do not provide suncream for my child.

I give permission for photographs/videos of my child to be used on the private Pre School Facebook Group.

I give permission for photographs/videos of my child to be used on the public YMCA Facebook page.

I give permission for my child to leave the Pre School premises with the Pre School staff to enjoy activities such as local walks, library visits, and visiting local schools.

I give permission for my child to participate in food tasting activities.

I agree to abide by all Pre School & YMCA policies and inform the Pre School of any significant changes in my child’s life.

I will take an active part in my child’s learning and I understand that the Pre School will inform me of any changes or relevant information about my child.

I give permission for YMCA Cheltenham and Pre School staff to take photos of me, my children and their artwork which will be kept under password and used without names for publicity purposes, including literature, Social Media and the Website.

Any information regarding you and your family is kept confidential and will not be passed to organisations outside of YMCA Cheltenham without your consent, unless it is of a Child Protection nature, in which case information will be shared with appropriate agencies. Pre School staff have a duty to ensure the safety and protection of children and we have formal procedures we are legally obliged to follow, if there are concerns about the wellbeing of children. Pre School undertakes steps to keep all information in a secure location.
